A: Please call the office at (386) 274-0250 anytime Monday thru Friday from 8:00 a.m. to 4:30 p.m. If you have a primary care provider or referring physician ensure that they have sent the necessary paperwork required for your visit.
Q: What do I need to bring to the appointment?
A: Please bring any records, prior test results, and films, if your referring doctor didn’t already send them to us. Also, you may print the patient forms on this site and complete them prior to your visits. Ensure you have your insurance cards and a photo ID with you at the time of your visit. If your insurance requires a referral and or authorization, please make sure your primary care doctor has obtained one for you to see the doctor.
Q: How do I get the New Patient forms that need to be filled out for my appointment?
A: Please go to the “Patient Forms” tab under Patient Resources and print the forms. Please bring the completed forms to your appointment along with your insurance cards and a photo ID.
Q: What insurances do you accept?
A: Volusia Surgeons, LLC accepts most major health plans. Volusia Surgeons, LLC may participate in some, but not all, products offered by a health plan. Please contact our office at (386) 274-0250 to discuss specifics of your insurance plan with one of our staff members.
Q: Does your office see “same day” patients?
A: We always do our best to accommodate requests for “same day” evaluations, typically with an urgent referral from a primary care provider, urgent care, or emergency room. As long as permitted by a patient’s insurance plan, an individual can request a same day appointment.

A: Volusia Surgeons, LLC has privileges at Twin Lakes Surgery Center, Halifax Health Medical Center of Daytona Beach, AdventHealth Daytona Beach, AdventHealth New Smyrna, and AdventHealth DeLand.
Q: Why am I receiving bills from North Florida Surgeons, P.A.?
A: Volusia Surgeons, LLC is part of a larger group known as North Florida Surgeons, P.A.
